Patients and methods
All cases of lung transplantation that were performed between October 1, 1987 and June 30, 1997, and which were reported to the UNOS/ISHLT registry, were analyzed. Because the focus of this analysis was the impact of donor age and ischemia time on outcome, only transplant cases with known donor age and ischemia time were included in this analysis (n = 5052). A total of 136 lung transplant centers participated in this study, including 88 from the United States and 48 from other countries.
Patient
Univariate analyses
The actuarial survival of lung transplant recipients according to the total lung graft ischemia time is shown in Figure 1. This figure includes data on 830 patients who received a lung graft that was subjected to greater than 6 hours of ischemia and 384 patients who received a lung graft that had been ischemic for over 7 hours. Actuarial survival was not significantly different according to the total lung graft ischemia time (p = 0.40).

Discussion
The results of this study show that in a large multinational cohort of patients undergoing lung transplantation over a 10-year interval, the total lung graft ischemia time was not associated with early postoperative mortality. This finding is in marked contrast to heart transplantation, in which increased graft ischemic time was a statistically significant risk factor for 1 year and 5 year postoperative mortality.2 In the current study, only the quadratic power of ischemia time (but not
