Aim: To evaluate the prevalence and types of PFT abnormalities in alloHCT patients and their association with Graft-versus-host disease (GVHD).Methods: Retrospective evaluation of 53 alloHCT recipients aged 39.4±15.2 years, 60.4% males, who underwent PFT before and 6-12 months after alloHCT. Forced expiratory volume in 1 s (FEV1), forced vital capacity (FVC), FEV1/FVCratio, Total Lung Capacity (TLC), Residual Volume (RV) and lung diffusion capacity for carbon dioxide (DLCO) corrected for the hemoglobin levels were determined.Results: A statistically significant reduction was noted in FEV1 (pre-alloHCT of 99.6%±21% vs. post- alloHCT of 88.4% ± 24%, p<0.001), FVC (pre-alloHCT of 97.1%±16.7% vs. post- alloHCT of 87.4% ± 20.4%, p<0.001), TLC (pre-alloHCT of 89.1%±13.7% vs. post- alloHCT of 88.3% ± 16.2%, p<0.002), DLCO (pre-alloHCT of 75.4%±16.7% vs. post- alloHCT of 68.4% ± 24%, p<0.006). An increase in RV/TLC was noted (pre-alloHCT of 88.8%±18.4% vs. post- alloHCT of 96.8% ± 23.8%, p<0.02). The ratio of FEV1/FVC did not change significantly (pre-alloHCT of 83.6%±9.2% vs. post- alloHCT of 81.5% ± 10%, p=0.052). In GVHD patients significant differences in FVC, FEV1, MMEF, PEF, TLC, RV, RV/TLC and DLCO were found pre and after alloHCT whereas in non GVHD only in FVC, FEV1 and MMEF.Conclusions: PFT should be included to standard periodic examination before and after alloHCT procedure as the lung is a target organ for GVHD.
