Fernandes AU - Daisy Abreu AU - Maria Gabriela Andrade AU - Carolina Constant AU - Duarte Malveiro AU - Manuel Magalhães AU - Inês Azevedo AU - Eduarda Sousa AU - Rizério Salgado AU - Teresa Bandeira Y1 - 2014/09/01 UR - http://erj.ersjournals.com/content/44/Suppl_58/P1257.abstract N2 - We assessed the perspectives of pediatricians (Peds) and general practitioners (GPs) on definition and clinically important outcomes in bronchiolitis.The ABBA study was a nationwide online survey with members of the Portuguese Society of Pediatrics and of GPs' mailing lists.The questionnaire evaluated agreement with statements on bronchiolitis definition,and importance given to different outcomes.We used principal component analysis (PCA) to explore dimensions underlying disease definition,and MANOVA to explore their determinants.514 Peds and 165 GPs participated (59% specialists).On definition,most Peds agreed with preceding coryza (93%),wheezing (93%) and rales/crepitations (90%),and disagreed with sudden onset (68%),but varied on nr of episodes (1st: 32%,<3: 47%) and age limit (<12M:14%,<24M:76%).GPs less frequently agreed on all parameters except wheezing (χ2,p<0.01),many considered independence of nr of episodes (56%) or age (17%).Less than 5% physicians agreed with a common clinical trial definition (<12M,1st episode,wheezing).We retained three dimensions on PCA:one based on coryza,rales/crepitations and no sudden onset;another on nr of episodes and age;and one based on wheeze.Dimensions varied with specialty and level of training (p<0.01).The top 3 rated outcomes were:hospital admission and respiratory distress (Peds and GPs),and need for O2 therapy (Peds) or changes in lung function (GPs).Physician definitions of bronchiolitis have considerable variability and often mismatch those of clinical trials.Rating of important outcomes was consistent.Our results contribute to rational study design and interpretation of evidence,and inform a core outcome set in this field.
