Pulmonary involvement is the most important guide in predicting prognosis. While investigating the etiology of ILD in the chest diseases outpatient clinic, the characteristics of three subjects diagnosed with ASS were examined (Figure).NSIP was observed in two cases and organized pneumonia in one. Serum autoantibodies were positive against aminoacyl transfer RNA synthetases. Lymphocytosis was observed in BAL. All of them were evaluated by the multidisciplinary team and immunosuppressive therapy was initiated for the patients. Clinical and radiological improvement was detected in all cases during follow-up. Unfortunately Case 2 died in the 3rd year of his follow-up due to metastatic colon adenocancer.ASS is a clinically heterogeneous form of idiopathic inflammatory myopathy characterized by myositis, arthralgia, Raynaud's phenomenon, mechanic’s hand, ILD, and serum autoantibodies to aminoacyl transfer RNA synthetases. NSIP and organizing pneumonia are the most common forms of ILD. The prognosis is poor in the presence of ILD.
