Its treatment is mainly surgical. We herein describe the clinical and para-clinical varieties of ACCs as well as their surgical treatment and prognosis.Methods: Seven patients were operated in our department between 1994 and 2020 presenting a tracheo-bronchial tumor and whose definitive pathological examination concluded with an ACC.Results: There were 2 men and 5 women with an average age of 44 years. Dyspnea (5 patients) and non productive cough (5 patients) were the most frequent complaints. Bronchoscopy and computed tomography revealed a tracheal lumen narrowing at the proximal (2 cases), middle (1 case), distal and carinal (5 cases) part of trachea. Bronchial biopsies confirmed the diagnosis of ACC (2 cases). The surgical approaches were a Kocher cervicotomy (3 patients), a left hemi-Clamshell approach (1case), a posterolateral thoracotomy (2 patients), a video-assisted mini-thoracotomy (1 patient) and a sternotomy (1 patient). The procedures performed were a tracheal resection and reconstruction (4 cases), an enlarged left pneumonectomy to the carina and the trachea (1 case), a lobectomy (1 case), a tumor biopsy (1 case), and an exploratory thoracotomy (1 case). Two patients had positive resection margins. Seven patients received adjuvant radiotherapy. After a median follow-up of 5 years for survivors, one patient had died of pulmonary metastasis.Conclusion: Complete surgical resection of ACC coupled with radiotherapy provide the best chance of prolonged survival.
