TY - JOUR T1 - Diagnosis of Histoplasmosis Using EBUS-TBNA In Patients with Suspected Lung Cancer: Case Series of a Reference Center in Brazil JF - European Respiratory Journal JO - Eur Respir J DO - 10.1183/13993003.congress-2020.3476 VL - 56 IS - suppl 64 SP - 3476 AU - Caroliny Samary Silva Lobato AU - João Pedro Steinhauser Motta AU - Amir Szklo Y1 - 2020/09/07 UR - http://erj.ersjournals.com/content/56/suppl_64/3476.abstract N2 - Histoplasmosis may be radiologically similar to lung neoplasm, with peripheral pulmonary nodule and mediastinal or hilar lymphadenopathy.The use of EBUS-TBNA is well established in lung cancer staging, but its role in the investigation of mediastinal lymphadenopathy due to infectious causes is still poorly understood.Objective:To describe a series of 12 cases of histoplasmosis with pulmonary and lymphnode involvement attended at a referral center in Rio de Janeiro,Brazil.Methods:Database review of the EBUS procedures performed between August 2018 and December 2019 in our institution was carried out, with search of the results of chest computer tomography images, pathology reports of the EBUS-TBNA, cultures and serologies for Hystoplasma capsulatum.Results:From 119 patients submitted to EBUS in the established period,49 patients had EBUS-TBNA material sent for direct examination and culture of fungi,12(10%) presented final diagnosis of histoplasmosis.All patients had pulmonary opacities associated with mediastinal or hilar lymphadenopathy.In 5 patients granulomas were reported in the cythopatological reports,in 5 cases it was possible to identify fungal structures by silver staining in cythopathology,in 5 cases the fungal culture of the EBUS-TBNA aspirates revealed growth of H. capsulatum and in 4 cases the serology for histoplasma were positive.The use of EBUS was decisive for the diagnosis in 10 of the 12 reported cases.Conclusion:Histoplasmosis should be considered in the differential diagnosis in suspected lung cancer in incident areas of infectious granulomatous disease.
