TY - JOUR T1 - Transtracheal closure of bronchopleural fistula following postpneumonectomy with atrial septal defect occluder JF - European Respiratory Journal JO - Eur Respir J DO - 10.1183/13993003.congress-2020.1711 VL - 56 IS - suppl 64 SP - 1711 AU - Zili Meng AU - Jia Meng AU - Ying Yang Y1 - 2020/09/07 UR - http://erj.ersjournals.com/content/56/suppl_64/1711.abstract N2 - Purpose: To evaluate the clinical efficacy of an atrial septal defect occluder for managing bronchopleural fistula.Method: A 57-year-old man, who was revealed a fistula in the right main bronchial stump,was perfotmed with atrial septal defect occluder.The procedure of the device placement was performed by a bronchoscope with the patient under topical anesthesia. After local anesthesia with 2% lidocaine, the bronchoscope was introduced in the right bronchial stump. A guidewire was passed through the working channel of the bronchoscope into the fistula. A 7-F introducer deliver system with the closure device was passed over the guidewire into the bronchus. Then, the closure device was released successfully by pulling back the introducer sheath under bronchoscopic vision. The proximal portion on the bronchial side and the distal portion in the pleural space were both deployed successfully.Result: After the placement of the device, the patient showed immediate improvement of the symptoms. One months later, followup bronchoscopy revealed that the BPF was completely sealed off with granulation tissue. Conclusion: Transtracheal closure of the BPF with a atrial septal defect occluder is a safe, effective, and feasible approach.
