Aim of our research was to describe 5 cases of CiOP.Methods: 5 patient (4 male, 1 female) with the age ranged 41 -57 yrs with pathologic pattern of CiOP were included. We analyzed CT changes and pathologic features.Results: Patient complaints were cough (5/5), dyspnoe (5/5), weakness (2/5). On CT scan of chest there were ground glass opacities with consolidation and cavities (1/5), perilymphatic dissemination (2/5), cavity with perifocal infiltration (1/5) or interlobular septal thickening with ossification (1/5). Histological examination revealed linear masses or nodules of dense furious tissue, some of them were ossified. Recognizable OP with prominent eosinophil infiltration was found only in 1 case which looked like organized eosinophilic pneumonia. In two cases there were loose perivascular granulomas composed of histiocytes and lymphocytes. In two cases we saw atypical adenomatous hyperplasia and mucous hyperplasia of alveolar epithelium. And in one case dendriform ossification was more prominent than fibrotic bands and masses, in this case there was progression on CT scan during the half of year.Conclusion: In cases of CiOP we did not always see granulation plugs in the lumena of respiratory bronchioles and alveolar ducts. Ossification is almost always can be seen in such patients. We propose that CiOP is a reparative reaction which could be result of various lesions.
