Surgery is known to be difficult, with high postoperative morbidity rates. The purpose of our study was to assess the outcomes of patients managed surgically for aspergilloma.Methods: We retrospectively reviewed the records of 84 patients of pulmonary aspergilloma which were managed at the thoracic surgery department of Abderrahmen Mami Hospital Ariana - Tunisia.Results: The mean age was 51.69, with a male predominance of 52 (61.90%). The main symptom was hemoptysis. History of pulmonary tuberculosis was found in 18 cases (21.42%) and bronchial dilatation in 9 cases (10.71%). Aspergilloma was on the right side in 45 cases, left in 38 cases and bilateral in 1 case. Surgical procedures performed were: anatomical resections in 69 cases and atypical resections in 15 cases. Complications reported in the postoperative course were: hemorrhage in 2 cases, prolonged air leak in 7 cases, sepsis in 6 cases, 3 cases of atelectasis, one case of chylothorax and death in 3 patients.Conclusion: Despite the advent of new medical treatments, surgery is still a standard in the management of pulmonary aspergilloma. Postoperative outcomes are highly influenced by the patient’s status and the complexity of the surgery.
