As a rule these patients at the time of detection of the desease nave negative bacterial excretion. Hoverever, in Russian Federation there is no consensus regarding the necessity of surgery for LT.Our purpose is to prove the importance of surgery in patients with LT. We examined 153 patients with primary diagnisis of LT before and after surgery was perpormed . The eŃ…amination of surgical material included morphological, micribiological, moleculae-genetic (PCR in real time) stidies.Results: Out of 153 23 patients after resection were found non tuderculosis deseases:malignant tumors (11), hamartomas (7), anthracosis (2), Wegener granulomatosis (1), focal pneumonia (1), aspergillosis (1). In 11/23/48% cases the unjustified anti-tuberculosis drug therapy was administrated within of 1 to 6 months.The diagnosis LT was confirmed in 78,8% of patients, in 61,4% MBT was found (29,4% in sputum), in 57,7% - MDR and XDR TB was detected. Drug administration more, than 8 months in 26/153 hatients led to 12,6 times rising of thr expression of the MDRI gene, which is responsible for the syntesis Pgp protein. Its indicates the formation of resistance of lung cells to drugs such as rifampicin which was initiale obtained to 63,4% of patients.Conclusions: Examination of surgical material ensures verification of the diagnosis, determination of MDR/XDR MBT and lung cells, creates conditions for adequate chemotherapy.
