Our previous study of the microbiomeanalysis using bronchoalveolar lavage fluid (BALF) samples in community-onset pneumonia (COP) patientsshowed that a detection of anaerobes in the lung was not related to aspiration risks. The etiological causes of aspiration pneumonia are known to be related to poor oral hygiene in addition to swallowing dysfunction estimated as aspiration risks. We aimed to evaluate the relationship between anaerobic detection from BALF and oral hygiene status in COP patients.Patients and Methods: The microbiome in the BALF samples obtained from 34 Japanese COP patients between August 2013 and December 2017was evaluated, and oral hygiene including tongue coating score, and Oral Hygiene Index (OHI) were simultaneously evaluated. The lung microbiome using BALF was analyzed according to the clone library method using amplified partial fragments of 16S rRNA genes with universal primers. The relation between oral hygiene and periodontal conditions and anaerobic detection in BALF using microbiome analysis was examined.Results: The Poisson regression analysis showed that tongue coating score was significantly related with anaerobic detection (3 vs 0, incidence relative rate (IRR) 2.919, [95% confidence interval 2.215-3.845], p<0.001).Discussion and Conclusion: According to our results, poor oral hygiene may be associated with a detection of anaerobes from the lower respiratory tract in patients with COP.
