We looked at the problem from COPD.Material and Methods: We studied 38 smokers with COPD II-III stages (age–61.4±3.8 yrs, pack/year–28.8±4.3) repeatedly (3-8 yrs) in stable phase on adequate therapy. Measurements included clinical status, spirometry with bronchoobstructive reversibility test (BORT) and allergic markers: blood eosinophil level (E), common IgE level (cIgE) in plasma, Phadiatop (Ph) in serum. Each patient underwent from 3 to 9 (6.2±1.4) visits to the clinic (there were 240 visits).Results: Two groups (gr.) were identified: 1st–24 (63.2%) COPD pts with positive BORT at least once (in 18 (47.4%) pts BORT was positive repeatedly–almost in half of cases (from 2 to 6 times); 2nd–14 (36.8%) COPD pts with negative BORT on all visits. Allergic markers are presented in the table.View this table:Conclusions: More than 60% of COPD pts at least sometimes have a positive BORT, about 50% pts have it repeatedly. A new COPD phenotype may be called “COPD with a positive BORT”. E as an allergic marker have low sensitivity. Those pts who, in addition to a positive BORT, have an increase in at least one of the allergic markers, can be classified as “COPD-A overlap” (CAO) (unlike “A-COPD overlap” (ACO)).
