Venous involvement may cause venous thromboembolism (VTE) and arterial involvement results in aneurysms. Since VTE is a result of inflammation in BD, immunosuprressive therapy rather than anticoagulation is recommended.In this study, we retrospectively investigated the clinical characteristics and outcome of BD patients who had vascular involvement between 2000 to 2017.There were four male and 27 female BD patients with vascular involvement. Median age was 30 years. Three patients had a family history of BD. 77.4% of patients had deep vein thrombosis (DVT) and 45.2% had pulmonary embolism (PE).Six patients (19.4%) had pulmonary artery aneurysm. In all of these patients, aneurysms disappeared with immunosuppressive therapy. Four patients had massive hemoptysis. Two of them responded to immunosuppressive therapy. However, two patients required pulmonary artery embolization.67.7% of patients had anticoagulation, 74.2% corticosteroid, 74.2% azathioprine, 32.3% cyclophosphamide treatment. 64.5% of patients had recurrent VTE, some of them more than once. Three patients died. One due to metastatic cancer, one due to soft tissue infection and sepsis. And the last one possibly due to recurrent PE.As a conclusion, BD patients with vascular involvement usually present with DVT. Immunosuppressive treatment is effective for aneurysms and hemoptysis.
