The great majority of sequestrations occur within the lower lobes, more often on the left than on the right.Methods: We present a case of 70-year-old female admitted to our institution with 3-months history of cough and progressive hemoptysis. Physical examination and laboratory data were unremarkable. Chest radiography showed two posterior homogenic mediastinal masses. In addition, computer tomography of the chest revealed athelectasis of the dorsobasal segment of right lower lobe with abnormal vessel from distal descendenting part of thoracic aorta, suggestive of pulmonary sequestration.Results: The progressive haemoptisis was the indication for surgery in this case. The patient underwent standard right lateral thoracotomy due to dense adhesions, revealed preoperativelly by chest sonography. A right lower lobectomy (one abnormal vessel from distal descendenting part of thoracic aorta was found) and resection of the MBC cyst were carried out. Pathological examination showed abscess formation within the sequestration and bronchogenic cyst filled with dark green fluid. The postoperative period was uneventful and the patient was discharged at day fifth. One year after the operation, she is in good general condition, without complaints.Conclusions: Although very rare, the IPS should be suspected in adult patients who suffer from hemoptysis. According to the literature review, our patient is the oldest to be reported to have coexisting right IPS and MBC.
