Psychological aspects like anxiety and depression are powerful influences with regard to the general outcomes of PR, necessitating more detailed analyses in order to further optimize individualized PR.Aim: To assess the effects of PR on anxiety and depression in (sub-)clinically distressed patients with sarcoidosis.Methods: Analysis of the outcomes of consecutive patients after PR with HADS scores ≥ 8 or ≥ 11: compare baseline to one year follow up values in anxiety and depression, including standardized response means (SRM) and their confidence intervals; document patients achieving clinically relevant improvement (2 point reduction) over time; multilevel modeling of changes will be presented at the conference.Results: Out of n=296 patients, n=117 patients with relevant anxiety and n=105 with relevant depressive symptoms sustained reduction of anxiety/ depression one year post PR (p≤.001; SRM 0.39; 95% CI 0.21-0.57); (p≤.001; SRM 0.47; 95% CI 0.28-0.66). A proportion of 27.3% and respectively 40% of the anxiety /depression subsample reported clinically relevant symptom reduction. The effect of PR is more pronounced in severely distressed patients (HADS≥11): 47.4% respectively 57.1% patients reported clinically relevant symptom reduction (Anxiety: n=38; SRM 0.64; 95% CI 0.32-0.95; Depression n=35; SRM 1.03; 95% CI 0.69-1.36).Conclusion: Multidisciplinary PR seems to have a long lasting positive effect on psychological distress in patients with sarcoidosis even one year after PR.
