Using the digital allows to make educational innovations. W'ASM (Web Assisted Asthma Management) is a video solution for automatic analysis of patients taking their inhaled treatment.Aims and objectives: Validate the W'ASM solution by comparing the results of the automatic analysis algorithms to expert judgment, in patients with asthma and healthy subjects.Methods: The different stages of the pressurized metered dose inhalers utilization technique (agitation, pre intake expiration, inspiration and apnea post intake) were modeled, from video recordings webcam. The movement of the inhaler, the movements of the head and chest were recorded and analyzed in real time. The algorithms were interfaced to a software that can be used on a computer or on a tablet with a webcam. Their metrological characteristics were evaluated from 100 video recordingsResults: Video analysis is provided in less than 30 seconds, with comments on steps that were poorly made. The results show satisfactory sensitivity and specificity for the algorithms ranging from 70% (95% CI: 60.4% - 79.6%) to 93% (95% CI: 87.9% - 98.1%) and 70% (CI 95%: 60.4% - 79.6%) to 88% (95% CI: 81.2% - 94.8%) according to the different steps.Conclusion: Digital technologies can provide new solutions to improve the terms of inhalation pressurized metered-dose aerosol. A clinical validation study is underway on more than 100 patients, recruited from hospital consultations of pneumology, with a gold standard consists of the evaluation by two pulmonologists experts (and a third if discordance).
