Eur Respir J 2013;42:636-46).Methods: We analyzed 74 patients with COPD (63 males, age 70±8 years, 55 ex-smokers, post-bronc FEV1% 51.8±19.6 %pred) who attended to a pulmonary rehabilitation program. All patients, in a stable phase of the disease, underwent complete clinical and functional assessment and sputum induction for differential cell count. Regular treatment was withdrew 24 hours before examination; none was under regular oral corticosteroid. According to 2011 GOLD Guidelines, patients were stratified in four A, B, C, D categories. Results: Main differences between the four groups are reported in the table.GOLD 2011 categoriesABCDp valuePatients, N 7413 (18%)5 (7%)20 (27%)36 (48%)TLC, %105.4±9.4131.7±16.0120.6±24.8131.4±30.3= 0.02DLCO, %86.3±16.353.6±19.591.3±27.653.7±19.7<0.0001Neutrophils, %66.5 (38-91)74.9 (71-96)72.1 (18-88)78.8 (58-97)= 0.03Eosinophils, %2.4 (0.4-21)1.4 (0-3.5)2.9 (0-60)0.9 (0-23)= 0.02mean±SD; sputum cells: median (range)TableGroups A and C show better quality of life (by Saint George Respiratory Questionnaire) and higher sputum eosinophils, while groups B and D had higher TLC, lower DLCO and higher sputum neutrophils. Conclusion: 2011 GOLD categories may identify different COPD phenotypes (prominent chronic bronchitis vs emphysema).
