However, the existing studies have not focused on body typeThe objective of our study was to determine if the android/gynoid fat ratio is positively correlated with prevalence of OHS and to develop a scheme of NPPVProspective study in OHS patients performing NPPV over a period of 1 year. Patients were divided into 2 groups according to the type of fat ratio. We also compared the efficacy of NPPV in supine patients with those in the lateral position82 patients were eligible for analysis with a mean body mass index 45,3+-7,7 kg/m^2. 43 patients were included in the group of android fat ratio (AFR) and 39 in the group of gynoid fat ratio (GFR). 42 patients were included in group of supine position and 40 in the group of the lateral position. PaCO2 in the AFR group was significantly higher than in the GFR group (P<0,05) especially in women (P<0,01). The initial IPAP and EPAP in supine patients were significantly higher (P<0,01) comparing those in lateral position. There were no differences between groups in gas exchange improvement. The duration of ventilator use was higher in group of lateral position than in supine patients (P<0,04)We conclude that the android fat ratio is closely associated with prevalence of OHS especially in women. NPPV in the lateral position requires smaller IPAP and EPAP increment versus NPPV in the supine position. ER -
