Cannabis contains substances similar to those found in tobacco. For this, we conducted a comparative prospective study spanning two years, and since January 2012 to December 2013. We compared two groups: group 1 patients who use cannabis alone and group 2 patients smoking only tabacco and both have lung cancer confirmed histologically. In the 2 groups there was a male predominance. Radiologically, it was a peripheral parenchymal process in 67% and 75% respectively in group 1 and 2. endoscopically, the most common aspects were respectively in the two groups, endobronchial tumor buds (25 and 34% of cases) and infiltrative stenosis (20 and 15% of cases). Histological confirmation is provided by bronchial biopsy in 20% (group 1) and 37% (group 2). And transthoracic biopsy in 25 and 19% respectively. Among histological subtypes, we found in the 2 groups, adenocarcinoma in 60 and 57% and the squamous carcinoma in 35% and 30%. According to the TNM classification, we have recovered in the two groups, stage III in 60 and 67% and stage IV in 35% and 32%. The treatment was surgical in 20% case in group 1 and 25% of patients in group 2. Forty percent of the patients were put under symptomatic treatment in group 1 and 35% in group 2. This study shows that the table clinical radiological and histological, does not differ that it was lung cancer in cannabis or only smoking. Nevertheless, it is important to recommend stopping cannabis. %U
