NBI bronchoscopy allow improving the early detection of preinvasive bronchial tumors.Aims and Methods: The aim was to determine the effectiveness of NBI bronchoscopy in detection and characterization of negative X-ray bronchial lesions. 204 patients with suspected lung cancer were enrolled in this study. Initially all patients were investigated by chest x-ray, then patients were examined by bronchoscopy (Olympus Exera II BF 1T180). The vasculature of lesions was assessed by using NBI mode and classified into four types:“tortuous”, “dotted”, “tortuous dilated”, “spiral and screw”. Biopsies were taken from all lesions for histological assessment.Results: From 204 patients 58 central lesions which were not detected by X-ray were identified by bronchoscopy with NBI. From 58 lesions were found: 12 squamous metaplasia (SM), 16 high-grade dysplasia (HGD), 9 carcinoma in situ (CIS), 8 invasive carcinoma (IC), 13 benign lesions. “Tortuous” were found in 27 cases (11(91,67%)SM, 8(50%) HGD, 3(33%) CIS, 10(85%) benign lesions); “dotted”, “spiral and screw”, “tortuous dilated” – in 31 cases (1(8,3%) SM, 12(75%) HGD, 9(100%) CIS, 8(100%) IC, 2(15%)benign lesions). Sensitivity, specificity, accuracy, were 97%, 92%, 94,8% respectively.Conclusion: NBI bronchoscopy is useful for detection and characterization of negative X-ray bronchial lesions. The results of the study showed good correlation of vasculature pattern with histology: tortuous vessels are typical for benign lesions, dotted, spiral and screw pattern with tortuous dilated vessels are characterized neoplastic lesions.
