Abstract
Introduction: Home Respiratory Polygraphy (HRP) was developed to allow a polysomnography-equivalent diagnosis of Sleep Apnea Hypopnea Syndrome (SAHS), but with fewer channels. Although it is recommended to carry out a manual analysis of the studies, these devices allow an automatic and faster scoring. No comparison was made between analyzes with the Embletta MPR®.
Aim: To evaluate the agreement between automatic and manual analyzes in the diagnosis of SAHS by HRP.
Methods: This retrospective study included adult subjects who performed HRP with Embletta MPR® at São João University Hospital (Porto, Portugal) between May and September 2020; inconclusive or therapeutic studies were excluded. Each study was reviewed automatically, followed by manual analysis. The association grade between automatic and manual scoring was evaluated comparing Apnea-Hypopnea Index (AHI) and using Kappa coefficient.
Results: A total of 253 subjects were included, 63% males, with mean (sd) age of 56 (13.2) years old, 79.8% of them diagnosed with SAHS by manual scoring. There was a significant difference between automatic (aAHI) and manual (mAHI) apnea/hypopnea indexes (8.5 ev/h; p<0.001). The agreement between mAHI and aAHI was 78.1% for SAHS diagnosis, with a Kappa coefficient of 0.52, and 47.0% for severity classification, with a Kappa coefficient of 0.29.
Conclusion: This study corroborates the findings that, compared to the manual, automatic analysis of HRP underestimated the AHI values, which led to false negatives of SAHS in 21.9% and an incorrect classification of severity in 53.0% of patients.
