Abstract
Aim: to evaluate the role and effectiveness of long-term comprehensive pulmonary rehabilitation (PR) in patients with COPD.
Materials and methods: 178 patients (98 men, mean age 59,7 ± 4,2 years), has made 2 groups: 1-st - 87 patients in complex treatment which was included prolonged course (12 months) of the PR. 2-nd group - 91 patients who received conventional medical therapy, according to the stage of the disease observed in outpatient. For patients of the 1-st group, depending on the stage of COPD, comorbidity, lung function, have been developed individual programs of PR: 1) antismoking programs; 2) systems of breathing exercises and physical exertion; 3) nebulizer inhaled therapy; 4) massage of the chest; 5) psychotherapy correction; 6) educated course (about life stile, usage of inhaled devices); 7) regular monitoring of the another specialists and Pulmonary Function Tests (PFT). The efficiency of complex treatment and rehabilitation we evaluated by the dynamics of clinical symptoms, PFT, frequency and severity of the exacerbations, dynamic of QoL (SGRQ).
Results: the patients of the 1-st group had positive dynamics of the clinical and functional parameters in a 12 month (the tendency to reducing the number and severity of exacerbations, stabilization of the PFT data). After PR course of the general condition of the 1st group of patients was accompanied by a substantial increase in their QoL: the significant improvement scores on all scales of the SGRQ (greater than 4%).
Conclusion: programs of individual physical and psychological rehabilitation allow to reach the improvement of clinical parameters and patients QoL in COPD patients.
