Abstract

Vocal cords dysfunction is one of the unfamiliar pathological condition that require differential diagnosis between asthma in children, especially in elder children.

The study was conducted during 3 years from 2012 to 2014. Patients were hospitalized in the allergic department because of absence of asthma treatment effect.We observed 84 patients aged from 12 till 16 years (mean age 14,2 ± 1,8 years), 44 boys and 40 girls, who suffered from uncontrolled asthma, varying degrees of severity. The examination revealed discrepancy between the severity of asthma and complaints of patients and lack of basic effects and bronchodilator therapy. Was determined that 26 patients (30.9%), including 10 boys (38.5%) and 16 girls (61.5%), had dysfunction of the vocal cords, which was found by dynamic clinical observation and repeated spirographic research.

The gold standard for diagnosis of vocal dysfunction is a direct fibrolaryngoscopy, which clearly visualize discordant reduction of the vocal cords during breathing movements. Consider method invasiveness, certain technical difficulties of its use and presence in most patients manifestations of somatoform autonomic disorders, the proposed method was repeated in spirographic survey with subsequent analysis of the results, which were used in our investigaton. Figure loop “flow-volume” in this case has a specific look that can diagnose the presence of dysfunction.

Received data establish the availability of significant percentage of cases with dysfunction of vocal cords in the structure of uncontrolled asthma among older children. A frequent manifestation of vocal dysfunction is psychogenic cough.
