Abstract

Background: We reported that effects of cough mucus clearance on the differences of rheological property, driving pressure and frequency during HFCWO in ERS Congress 2013. The purpose of this study is to compare the efficacy of cough with MI-E and HFCWO.

Methods: Study 1:

Four MI-E equipments were compared the movement distance of the different viscoelastic mucus stimulants (MS) in tube length 1m internal diameter 1cm with varying compliance and resistance using a TTL model lung. We set adjustment by applying a driving pressure of three 30/-30, 40/-40, 50/-50cmH2O, I time1.5sec, E time1.5sec, pause time1sec. Then we measured the moving distance of MS with combination of percussion mode.

Study 2:

10 normal subjects (21.4±1.3yr) were also measured migration length of each MS by coordinated compressing chest strap in HFCWO by mask in MI-E.

Results: In the MS of 1%, the moving distance is increased as the pressure is stronger in the each MI-E (p<0.05). The moving distance is dependent on the flow rate of the device and resistance (p<0.05). On the other hand, in the MS of 4%, the moving distance was no difference due to equipment of MI-E. However, the migration length was increased significantly by the combined use of percussion mode in 4% MS (p<0.05). The moving distance of MS in combined MI-E and HFCWO were significantly greater than the MI-E equipment (p<0.05).

Conclusions: Moving distance of MS in the MI-E equipment was dependent on airway resistance and the flow rate of the device. Combination of percussion was effective in the high sputum viscoelasticity. Combined MI-E and HFCWO was more effective than applying only MI-E.
