Abstract

Aim: Analyze the complications of transbronchial cryobiopsy in patients with ILD

Methods: Prospective observational(January 2012-January 2015)54 patients by ILD.Transbronchial biopsies using cryoprobe(ERBE Flexible cryoprobe L900mm)in the operating room with the patient intubated. Severity of hemorrhage as mild,that permitted local maneuvers and does not include further exploration;moderate that requires the suspension of the exploration and severe that threat the life.

Results: Mean age 58,7±9;57% male. The average number of biopsies was 3(1-5)and 83% 3 or more. Total biopsies were 165. 80% contain lung parenchyma.100% of the patients obtained at least one valid sample. After appropriate radiological-clinical-pathological correlation a diagnosis of safety/high probability 87%.

Biopsies: RLL(68.5%). The mean duration54±19min.Average area of samples:20.92mm2(3-4). Pneumothorax was the most common complication,8,7 required pleural drainage.

Hemorrhage: 5 mild 66.6%.All patients were hospitalized 1 day after the exploration,6 of them none.4 with pneumothorax and 2 with hemorrhage.

Conclusions: 23% patients had complications.Pneumothorax was the most common complication in our environment followed by hemorrhage. We found not significant correlation between the location lobe biopsies and the risk of development of pneumothorax. Mild hemorrhage was the most frequent. The cryoprobe lung biopsy has a high diagnostic yield, although not without complications.
