Abstract
Studies show effective transition between child & adult care improves long term outcomes. RSG is a generic programme designed & implemented to address the challenges transition presents. Medical, psychosocial & vocational needs of individuals are addressed following a structured yet adaptable plan, supported by Young Persons Clinic weeks. RSG addresses issues common to all young people but allows the respiratory team to focus on individual's specific needs. It commences at 11years coinciding with transition to senior school. A key principle of the programme is to empower young perople to take control of their lifes & equip them with the necessary skills to function independently & confidently in the adult services

93 completed questionnaires
-40 patients
-31 parents
-22 professionals
Conclusions
RSG is an effective tool for delivering optimal transition to adult services.
