Abstract

Background.

In the 1950s Russian scientist V.P. Demikhov proved the opportunity of lung transplantation. Since that time the lung transplantation became the worldwide method of treatment for end-stage lung diseases. Annually above 3500 operations are performed in the world. And only 12 same operations were done in Russian Federation for last 50 years and only the one was successful.

Aims and objectives.

We evaluated the early and medium-term single-centre results for lung transplantation in N.V. Sklifosovskiy Research institute.

Materials and methods.

11 double lung transplantations have been performed from May 2011 to May 2013. Major indications included obstructive lung disease (3), idiopathic pulmonary hypertension (2), cystic fibrosis (2), idiopathic pulmonary fibrosis (2). There were 4 males and 7 females with mean age 34,5±6,9 years among all recipients. The extracorporeal support was used at 7 procedures: 1 – cardiopulmonary bypass and 6 – venoarterial pump-driven extracorporeal membrane oxygenation. The immunosuppression was proved with induction therapy and maintenance standard three-component therapy.

Results.

The 1- and 2-year survival rates in all recipients were 44,4% and 32,4% respectively. The early mortality rate was 36,4% (4). The causes of death were infection with airway dehiscence (2) and multisystem organ failure (2). The pulmonary infection was the principal cause for early postoperative complication. Severe primary graft dysfunction (3 grade) developed only in 2 recipients. There weren’t any deaths in the late stages of a post-transplant period.

Conclusions.

The early and medium-term results for lung transplantation are not bad. But an efficient system is needed to improve.
