Measurable reversibility is likely if the patient gives a history of considerable variability in shortness of breath demonstrated, for instance, by excellent exercise tolerance sometimes and marked limitation by dyspnea at other times. This is particularly significant when the increased dyspnea occurs in association with wheezing, especially when there is exposure to possible allergens. The history is important if the patient is seen during symptom-free period when examination of the chest and

Post-bronchodilator Reversibility of FEV<inf>1</inf> and Eosinophilic Airway Inflammation in COPD
2017, Archivos de BronconeumologiaCitation Excerpt :The fully adjusted OR was 4.262 (95% CI = 1.165–15.595, P=.029, Table 2). When subjects were dichotomized by FEV1 increment >15% from pre-dose values (ACCP criteria for positive bronchodilator reversibility),15 a subtle albeit statistically significant difference in the levels of sputum eosinophils was observed (1.78 vs 1.00%, P=.04, Table A2 in the appendix). ROC analysis showed an area under the curve of 0.58, P=.034 (Fig. 2).
Bronchodilator Response in FVC Is Larger and More Relevant Than in FEV<inf>1</inf> in Severe Airflow Obstruction
2017, ChestCitation Excerpt :The FEV1 increased by > 12% baseline in 20% of subjects with mild obstruction compared with 55% in very severe airways obstruction (Fig 3); this is inconsistent with structural changes of peripheral airways and lung parenchyma in fixed airways obstruction.22 This paradox arises because the absolute BD response in FEV1 is independent of the initial value (e-Fig 4A).3,6,7,9,23-25 Therefore, expressing change as % baseline spuriously suggests that the poorest initial values are associated with the greatest reversibility (e-Fig 4B); this is attenuated by expressing change as a percentage of the predicted value (e-Fig 4C).
Radiological correlates and clinical implications of the paradoxical lung function response to β<inf>2</inf> agonists: An observational study
2014, The Lancet Respiratory MedicineAssay of IL-22 and IL-25 in serum, whole blood, and peripheral blood mononuclear cell cultures of patients with severe asthma
2014, Allergologia et ImmunopathologiaCitation Excerpt :To identify the severity of asthma, a trained observer assessed airway reversibility, peak flowmetry, and spirometry in the asthmatic patients. At least three acceptable manoeuvres meeting the American College of Chest Physicians Standards were required, with the minimum of two reproducible forced expiratory volume in 1 second (FEV1) and forced vital capacity (FVC) manoeuvres within 5% of best required for each test.18 The airway responsiveness was performed according to a standardised protocol.19
