Abstract

Elderly-onset rheumatoid arthritis (EORA), defined as rheumatoid arthritis (RA) with onset at age 60 years or over, differs slightly at presentation from younger-onset RA (YORA) by a more equal gender distribution, a higher frequency of acute onset with systemic features, more frequent involvement of the shoulder, and higher disease activity. Longitudinal studies have showed more disease activity, radiographic damage, and functional decline in patients with EORA than in those with YORA. These differences were only found in seropositive patients. Seropositive EORA was reported to be associated with HLA-DR4, in contrast to seronegative EORA. Possible heterogeneity in the pathogenesis of seronegative EORA is supported by the recognition of subsets that overlap with the clinical manifestations of other syndromes such as polymyalgia rheumatica and remitting seronegative symmetrical synovitis with pitting edema. In addition, crystal-induced arthritis and inflammatory osteoarthritis may be difficult to distinguish from EORA. The efficacy and toxicity of second-line drugs is similar in both age groups, but in the elderly caution is needed with the use of nonsteroidal antiinflammatory drugs and prednisone.

Elucidating a bidirectional association between rheumatoid arthritis and depression: A systematic review and meta-analysis

Rheumatoid arthritis (RA) and depression are conditions which commonly co-exist. Recent longitudinal studies now suggest a bidirectional association between these disorders, with inconsistent results. We conducted a systematic review and meta-analysis to examine this relationship.
Three electronic databases (PubMed, Embase and PsycINFO) were searched from inception to September 4, 2021 for cohort studies evaluating either the risk of depression in RA patients or the risk of RA in patients with depression, as well as the secondary outcome of all-cause mortality risk in RA patients with depression. A random effects model was used to summarize the included studies.
Eleven cohort studies were included, comprising a total of 39,130 RA patients, 550,782 patients with depression and 7,802,230 controls. RA patients had a 47% greater risk of incident depression compared to controls, while patients with depression had a 34% greater risk of developing RA. Subgroup analysis by age was only significant in the ≥60 years old age group. RA patients with depression had an 80% increased risk of all-cause mortality compared to those without depression.
The results may have been confounded by factors such as differing methods of depression ascertainment across studies and overlap in presentation between the two conditions.
There exists a bidirectional association between RA and depression especially in the elderly which increases mortality risk. This invites the need for clinicians to screen and be vigilant for the presence of these conditions.
Effect of age at rheumatoid arthritis onset on clinical, radiographic, and functional outcomes: The ESPOIR cohort

Rechercher si l’âge d’apparition de la maladie conditionne l’évolution clinique, radiographique ou fonctionnelle dans une cohorte de PR débutantes.
La cohorte ESPOIR est une cohorte multicentrique des patients atteints de polyarthrites débutantes. Ont été sélectionnés dans notre étude les patients qui remplissaient les critères ACR/EULAR 2010 pour la PR pendant les 3 premières années du suivi. Ces patients ont été répartis dans 3 groupes en fonction de l’âge d’apparition de la PR : < 45 ans (PR d’apparition précoce [YORA]), 45 à 60 ans (PR d’apparition intermédiaire [IORA]) et > 60 ans (PR d’apparition tardive [LORA]). Les résultats suivants ont été comparés à l’inclusion puis au cours des 3 premières années de suivi : La rémission selon le score SDAI (Simple Disease Activity Index), la présence d’une érosion radiologique supplémentaire, le questionnaire HAQ-DI (Health Assessment Questionnaire Disability Index) < 0,5 et le taux de maintien thérapeutique du premier traitement de fond (DMARD–Disease-Modifying Antirheumatic Drug)
Un total de 698 patients a été inclus (d’âge moyen [écart interquartile] 50,3 [39,8–57,2] ans), 266 YORA, 314 IORA, et 118 LORA. 1 an, on notait une rémission SDAI plus importante pour les patients YORA que pour ceux des groupes IORA et LORA (p < 0,0001). La présence d’au moins une érosion radiologique supplémentaire intéressait principalement les patients LORA et IORA comparativement à ceux du groupe YORA, à 1 an (p = 0,009) puis à 3 ans (p = 0,017) de suivi. La proportion de patients ayant un score HAQ < 0,5 était plus élevée dans le groupe YORA que dans les groupes IORA et LORA à 1 (p = 0,007), 2 et 3 ans de suivi. Le taux de maintien thérapeutique du premier DMARD était plus faible dans le groupe YORA que dans les autres groupes au cours des 3 ans (p = 0,005) de suivi.
Dans une cohorte de PR débutantes, l’apparition précoce de la maladie est associée à un taux élevé de rémission à 1 an, une progression radiologique nulle à 3 ans et un faible score fonctionnel au cours des 3 ans de suivi.

A better characterization of differences between LORA and YORA prognosis could help rheumatologists with therapeutic decision-making in terms of medicine tailored to the individual patient. LORA is characterized by a more balanced gender distribution, a higher frequency of acute onset, and more frequent involvement of large joints than YORA [5–8]. Some studies have suggested genetic differences between LORA and YORA, with fewer older patients carrying HLA-DRB1 susceptibility alleles than younger patients [9].
To investigate whether age at disease onset determines clinical, radiographic or functional outcomes in a cohort of early RA.
The ESPOIR cohort is a multicenter cohort of patients with early arthritis. We selected patients fulfilling the 2010 ACR/EULAR criteria for RA during the first 3 years of follow-up. Patients were pooled into 3 groups by age at RA onset: < 45 years (young-onset RA [YORA]), 45 to 60 years (intermediate-onset RA [IORA]) and > 60 years (late-onset RA [LORA]). The following outcomes were compared at baseline and during the first 3 years of follow-up: Simple Disease Activity Index (SDAI) remission rate, one additional erosion, Health Assessment Questionnaire Disability Index (HAQ-DI) < 0.5 and first disease-modifying anti-rheumatic drug (DMARD) continuation rate.
We included 698 patients (median [interquartile range] age 50.3 [39.8–57.2] years), 266 YORA, 314 IORA, and 118 LORA. At 1 year, SDAI remission was greater for YORA than IORA and LORA (P < 0.0001). Having at least one additional erosion was greater for LORA and IORA than YORA after 1 year (P = 0.009) and 3 years (P = 0.017). The proportion of patients with HAQ score < 0.5 was greater for YORA than IORA and LORA at 1 (P = 0.007), 2 and 3 years. First DMARD continuation rate was lower for YORA than other groups during the 3 years (P = 0.005).
In a cohort of early RA, young age at disease onset is associated with high rate of remission at 1 year, no radiographic progression at 3 years and low functional score during 3-year follow-up.
