Hydrogen ions >62 nM, pH<7.2
Hydrogen ions 56–62 nM, pH 7.2–7.25 on two occasions 1 h apart
Pa,CO2 >8 kPa (60 mmHg) and Glasgow coma scale <8 (hypercapnic coma)
Pa,O2 <6 kPa (45 mmHg) despite maximum tolerated inspiratory oxygen fraction
Cardiorespiratory arrest
