Damage of alveolar epithelial barrier is a critical event in the early stage of ALI/ARDS. Alveolar type II cells (ATII) are implicated in the alveoli reparation and the transplant of these cells could be a promising ALI treatment.The objective is to determine the therapeutic role of ATII cells transplantion in an animal model of ALI.Methods: Sprague-Dawley rats were anesthetized with isofluorane. ALI was induced by intratracheal instillation of HCl (1μl/g, 0.1 mol/L) followed by instillation of LPS from Escherichia coli O55:B5 (30μg/gb.w.) 2h later. Control rats were treated with saline. 9h after the second instillation, one group of rats were intratracheally transplanted with fresh ATII cells (2.5x106 cells in 300μl).Results: HCl+LPS instillation caused an increase in lung weight (1.8±0.2g) vs control (1.3±0.2g), owing to the inflammatory component. ATII trasplanted group had a significantly reduction in lung weight (1.5±0.15g). Total protein (C:286µg/ml,HCl+LPS:626µg/ml,TRP:326µg/ml), IgM and neutrophil number (C:5•106cells,HCl+LPS:2,1•107cells,TRP:1.4107cells) in BAL were significantly increased in HCl+LPS group and the trasplant of ATII was able to reduce all of them (p<0.05). Hystological lung tissue analysis showed a recovery of the alveolar damage in ATII trasplanted animals versus the peribronchiolar and interstitial infiltration, inflammatory cells, interstitial edema and hemorrhage observed in ALI rats.In conclusion trasplant of ATII cells is able to reduce the pulmonary inflammation and injury.
