This is a prospective study. We studied 91 consecutive patients with COPD (mean age: 70.0±10.42 years). The subjects were divided into four groups (Healthy control, COPD without respiratory failure, COPD with success NIV and COPD with failure of NIV). Red cell distribution width was calculated by both impedance and flow cytometric analyzers as the part of the routine full blood count.Patients with COPD had significantly higher (p<0.001)RDW values compared to healthy control subjects (14.23±2.50 and 13.29±1.41). Patients with acute hypercapnic respiratory failure and NIV success had significantly higher RDW values compared to patients without respiratory failure (15.25±2.5 and 13.6±2.56) (p<0.005). The level of RDW was higher in patients with NIV success than patients with NIV failure (15.25±2.5 and 13.71±2.14) (p<0.05). Patients with COPD had significantly higher MPV values compared to healthy control subjects (p<0.001). But there was no difference between COPD patients groups according to MPV values.The presence of low RDW value was preedictor of NIV failure in patients with acute hypercapnic respiratory failure. The low RDW may be a marker of NIV failure.
