To accept extended donor lungs, one of the strategies to expand the donor pool, needs to improve its assessment. There is only 1 lung donor scoring system that included just 5 factors (Oto, T. et al. Ann Thorac Surg 2007; 83:257-264).The purpose was to develop more objective and full lung donor score (LDS).Data of all donors (n=69) referred to our Institute from 2011 to 2013 were divided into 2 groups: exclusive factors and estimation criteria. The estimation criteria consisted of PaO2/FiO2 ratio, data of chest x-ray and bronchoscopy (presence of purulent secretion and the endoscopic grade of bronchitis), bronchoalveolar lavage fluid cytology, duration of mechanical ventilation, age, smoking history, presence of blunt chest trauma. These criteria were weighted between 0 and 4. Data of transplant recipients for the same period were analyzed.42 (60.9%) donors were declined based on the exclusive factors. The aspiration of gastric contents was the principal causes for refusal (n=17; 40.5%) among them. 11 (15.9%) donors were used for LTx (U-group) and 16 (23.2%) were declined for medical reasons based on the estimation criteria (EC-group). The medial donors score in the U and EC groups were 5.0 and 13.9 respectively (t=-11.335, P<0.001). The score of 9 is a cutoff score (at 95% confidence interval). The 30-day mortality rate of recipients was 9.1% (n=1). Severe primary graft dysfunction developed only in 2 (18.2%) recipients.A new LDS takes into account most of all data of donor assessment and it could be more objective one. Our investigation has the limitation in use only brain dead donors, so an additional experience and prospective studies are required to define admitted LDS. ER -
