Both obstructive (OVD) and restrictive ventilatory defects (RVD) may occur after hematopoietic stem cell transplantation (HSCT) that are attributed to bronchiolitis obliterans (BO) or sclerodermatous cGVHD respectively. The aim of the study was to assess the ECP effect on pulmonary function testing (PFT) in patients with pre-ECP OVD or RVD. We analysed retrospectively the characteristics of all the consecutive HSCT recipients who were treated with ECP for skin cGVHD in our hospital between 2008 and 2012. PFT performed before HSCT, at the time ECP was started and at the time it was stopped, as well as when BO was diagnosed, were reviewed. 17 patients were included: 7 had an OVD (median Forced Expired Volume in 1 second (FEV1): 71% of predicted value [52.5; 75]). 5 had RVD (median Total Lung Capacity: 75% of predicted value [65; 75]) at the time of ECP. Median FEV1 for patients with BO measured before and after ECP were respectively 2.59L [1.73; 3.25] and 2.25L [1.67; 2.63] (p: 0.10); Monthly median FEV1 decline between the diagnosis of BO and the start of ECP and between the start of ECP and its stop were respectively +26.1mL [-4.4; +67.6] and -38.00mL [-152.85; -16.6] for patients with BO (p: 0.05). For patients with a RVD, the Forced Vital Capacity before and after ECP were respectively 2.09L [1.77; 2.37] and 2.11L [1.60; 2.89] (p: 0.81).Our results show no significant effect of ECP on the PFT of patients treated with ECP for skin cGVHD who also had an OVD or a RVD. However, these results need to be confirmed in a prospective study. ER -
