The DACCORD registry that collects data from a large real life population sample was used to categorise patients according to GOLD December 2011.MethodsData on spirometry, history of exacerbations, CAT and mMRC were collected from 6209 primary and secondary care outpatients with COPD. Patients with co-morbidities, long term oxygen use, or no smoking history were also included.ResultsAssessment based on FEV1 showed the following distribution: 11.7% mild, 51.4% moderate, 30.5% severe and 6.4% very severe. 36.9% had FEV1<50% and were assigned to C or D groups. Taking into account FEV1 and exacerbation history C and D comprised 53.4% of patients. This implicates that more patients are related to high risk due to their airflow limitation but not due to their history of exacerbations (figure). Based on mMRC <2, 38.8% of patients had few symptoms (A and C), using CAT <10, 9.5% were categorised into these 2 groups. Such differences in staging based on CAT vs mMRC are well known (Han, K et al. Lancet Resp Med 2013;1:43-50).ConclusionThe majority of patients were classified into groups C and D because of an FEV1<50%, while the exacerbation history categorised less patients into the high risk category. This is consistent with previous studies (Agusti, A et al. Eur Resp J 2013;42:1391-1401).
