AB  - Purpose: There are a few studies which analyzed many patients with pulmonary nontuberculous mycobacterial (NTM) disease complicated with pneumothorax. To clarify the clinical characteristics of patients with pulmonary NTM disease complicated with pneumothorax.Materials and Methods: Two hundred twenty patients satisfied the diagnostic criteria of NTM disease proposed by ATS. Nine patients with pulmonary NTM disease were complicated with pneumothorax. We investigated the background, laboratory findings, radiological findings, treatment and prognosis.Results: There were nine patients, 6 males and 3 females, with a mean age of 73.9 years old. Seven patients had underlying respiratory disease such as COPD. The causative microorganisms were Mycobacterium avium in 4 patients, M. intracellulare in 4, and M. kansasii in 1, respectively. Regarding the radioloical findings, pneumothorax was recognized in 5 patients in the right lung, 3 in the left, and 1 in both lungs heterogeneously. Although most patients showed multiple cavities and extensive lesion over unilateral lung fields, three patients were simultaneously diagnosed with pulmonary NTM disease at the onset of pneumothorax. As for treatment thoracic drainage was performed for seven patients, one was advised to rest only and one needed thoracic drainage and surgery. The responses to the treatment were poor and five patients died due to pneumonia or heart failure.Conclusions: The rate of pneumothorax complications in patients with pulmonary NTM disease (4.1%) was higher compared to other reports. The response to treatment was poor, and the prognosis was also poor due to other complications.
