End to end resection and anastomosis of the trachea is indicated in fibrotic stenosis, the surgery consists of a cervical dissection with exposition of the trachea, resection of the affected segment and joining of the ends without tension. Causes of stenosis include congenital illness, traumatism, intubation, surgery, infection, granulomatous disease and tumors. All patients present progressive dyspnea. It is diagnosed with a bronchoscopy and helicoidal tomography with 3-D reconstruction. Methods: We analyzed cases between 1996 and 2012 with diagnosis of tracheal stenosis with resection and end to end anastomosis. Results: Seventeen patients were diagnosed with tracheal stenosis and all of them were candidates for surgical treatment (100%), average age was 35.6, 13 men (76%) and 4 women (23%). The main etiology was endotracheal intubation of 16 patients (94%) followed by tracheal granuloma by foreign object in one of the cases (5%). The extension of the resection was from 2-5 tracheal rings. The complications were stenosis in one of the cases (5%) three months after surgery. One patient died (5%) as a consequence of a respiratory failure not related to the surgical treatment. Conclusions: Tracheal resection with end to end anastomosis is an excellent alternative with high success rate. This procedure is a surgical challenge performed in very little hospitals in our country and it requires third level infrastructure, training and experience of the surgeon. In Juarez City, Mexico we have had extremely satisfactory results with this surgical procedure.
