Differential diagnosis between primary and secondary melanoma is difficult. PM of bronchus has unfavorable prognosis. Four cases of PM of bronhus were investigated: males - 2, females - 2. Age of patients was 41, 53, 58, 72 years. Histological and immunohistochemical methods were used: hematoxylin and eosin stain, method of Fontana-Masson revealing the expression of S-100 protein, HMB-45, vimentin.Results. Histological examination in 3 cases revealed epithelio-like and polymorphocellular melanoma with alveolar type of growth, in 1 case - epithelio-like melanoma with sites of balloon transformation. Melanogenesis varies widely, one observation was represented by amelanotic melanoma. Immunohistochemical study in all cases revealed a positive expression of S-100, HMB-45 and vimentin. Primacy of bronchus melanoma in all cases was set to the exclusion of primary malignant melanocytic tumors at other sites (skin, mucous membranes of the nose, rectum), the presence of intraepithelial melanocytic proliferation in areas of bronchial epithelium metaplasia.Conclusion. Differential diagnosis of bronchus PM is complex and requires exception of metastatic melanoma of other localization. The crucial value has the primary tumor morphogenesis: staging of intraepithelial atypical melanocytic proliferation in areas of squamous metaplasia with development of melanoma foci in tumor field in situ).
