RT Journal Article SR Electronic T1 Video assisted medical thoracoscopy(VAMT) hot versus cold lung biopsy in diffuse lung diseases JF European Respiratory Journal JO Eur Respir J FD European Respiratory Society SP PA4670 DO 10.1183/13993003.congress-2016.PA4670 VO 48 IS suppl 60 A1 Tamer Ali El-Hadidy A1 Abdelhadi mohamed Shebl YR 2016 UL http://erj.ersjournals.com/content/48/suppl_60/PA4670.abstract AB Introduction Pathological diagnosis is an integral part of diagnosis of diffuse parenchymal lung diseases with failed diagnosis by less invasive facilities.Objective Evaluate safety and efficacy of VAMT lung biopsy by two different techniques hot lung biopsy versus cold biopsyMethods Prospective interventional study in Mansoura university hospital during 2014&2015, using VAMT lung biopsy comparing two groups as regard diagnostic yield and complication.Group A using grasping lung forceps connected to electrocautary(hot biopsy).Group B using rigid cryoprobe ERBE Germany (cold biopsy).Results Group A included10 patients from them 9 diagnosed and one not diagnosed (2 UIP-3 tuberculosis-2 adenocarcinoma-1 sarcodosis-1 lymphangitis carcinomatosis) with 90% diagnostic yield. Only 2 cases develop Broncho pleural fistula which need no surgical intervention, with biopsy specimen about 0.5 cm in diameterGroup B included 10 patients with diagnostic yield 100 %( 3 UIP-2 tubeclosis-2 adenocacinoma-2sarcodosis-1lymphoma) with no complication in this group, biopsy specimen about 1.5cm in diameter.Conclusion Lung sampling can be performed safely by VAMT with good diagnostic yield in cases of diffuse parenchymal lung diseaseUsing either hot or cold lung biopsy with better diagnostic yield and less complication in case of cryo thoracoscopic lung biopsy. Soon we hope it will represent good alternative to VATS and open lung biopsy.
