We developed the iPad (Apple) interactive app that conducts COPD-specific self-management education in 2014 (Am J Respir Crit Care Med 191; 2015:A1112). To assess the efficacy of iPad-based intervention in improving health status in patients with moderate to very severe COPD compared with usual care, a single-center RCT was started from June 2014. In addition to 40 educational contents, by default the program was individualized. The app assessed patient's symptoms and gave action plans when they had exacerbations. To reduce exertional dyspnea, motion pictures of stretching training were selected individually in addition to breathing retraining. The app encouraged patients to increase physical activity. Among 29 patients (age 70.3 ± 7.5 (SD), male 86.2%), 4 discontinued 16-week study. FEV1 of the intervention (n=14) and control (n=11) group were 1.21 ± 0.37 and 1.16 ± 0.36 L, respectively. The quality of life (SGRQ) and 6MWD were 16.0 (p=.005) and 48.6 m (p=.011) greater in the intervention group than in control group at 16 weeks, respectively. Between-group difference in LINQ was 9.7(p=.005).There were no withdrawals during the use of iPad in the intervention group. The individualized interactive approach and availability of motion pictures will enhance patient's adherence to the program by making learning fun. It is suggested that the tablet technology will be of value in improving health status of COPD patients and also be beneficial in spreading COPD-specific self-management education.
